Ishprret Ji - What a great question. Everyone has voices in their head: Your mom's voice, the voice of your own conscience, etc. Prayer is talking to God. Meditation is listening to God. When you deepen your listening, you go beyond the mental. conceptual realm. When you establish regular meditation/japa/paath then you gain depth and the ability to sort out your own mental processes from Direct Divine Guidance. In sum, if you don't know, then there's no indicator to point out. When you know, there's no doubt.
